Below you will find the components which make up a March of the Living application as well as information on other items which will need to be completed.. Please read the information carefully.  Use the list below as a checklist to be sure everything is sent in.  You may download the forms, complete them and mail them to the address listed or, you can complete them online and send them as an attachment in an email to rbaltuch@aol.com.

Considering the number of applications we anticipate, it is to your advantage to apply early.  Any application received after June 30th will be accepted on a space available basis and if necessary placed on a waiting list

  GENERAL INFORMATION/INSTRUCTIONS - please read first

*TEEN APPLICATION  - filled in completely, with signatures in appropriate places (passport number, 
   expiration date and country of origin may be submitted at a later date, but acceptance cannot occur until
   receipt of the information.) 

*DEPOSIT - $500 to accompany application.  You may send a check separately if completing online. 
  Visa/MC acceptedCredit card form available here 

*PICTURES -  we will need 2 passport pictures submitted by mail or a digital passport size picture.

*MEDICAL FORM - to be completed by a physician not related to you.  The first 2 pages are to be completed
  by the applicant.  The other pages completed by the physician.  The form should be sent directly to us by the
  physician via mail only. 

*RECOMMENDATION FORM - to be completed by a teacher or youth worker.  The form should be sent
  directly to us via mail or email attachment. 

*CANCELLATION & REFUND POLICY - to be read and signed by parent(s)

*FEE SCHEDULE - to be read and signed by parent(s)

CALENDAR - For your information.  Be sure to clear these dates on your calendar

FINANCIAL ASSISTANCE APPLICATION   Scholarships are need based. Payment plans are available. The deadline for filing for Financial Assistance is Thursday, September 15, 2011. In the past, the only qualified teen who did not go, was the one who did not apply


ORLOFF CAJE MEMBERSHIP INFORMATION - Each family must become a member of Orloff CAJE in the amount to the
  best of their ability.

* These items are necessary before an interview can be set up.

In addition, we will need a copy of your health insurance card as well as a copy of your passport ID page.
